Job Description

**Responsibilities** This individual provides physical therapy services under the direction of the Physical Therapist and according to the physician's plan of care. + Provides direct physical therapy according to directions of Physical Therapist and in accordance with the physician's plan of care. + Treats the patient to relieve pain, develop or restore function and maintain maximum performance. + Directs and aids patients in active and passive therapeutic exercises, muscle re-education, gait and functional training, transfer activities and prosthetic training. + Utilizes physical agents, including, but not limited to, heat, cold, water, light, electricity, ultrasound and massage, appropriate to the home setting. + Observes and reports to the Physical Therapist and the agency personnel, the patient's reaction to treatment and any changes in the patient's condition. + Instructs patients in the care and use of wheelchairs, braces, crutches, canes, and prosthetic or orthotic devices. + Maintains accurate and complete agency-required documentation and other necessary records, clinical notes and conference notes which will be incorporated into the patient's clinical record within defined time frames. + Participates in Home Health team meetings and inservice education programs. + Instructs patient's family/caregiver and other agency health care personnel in patient's treatment regime when indicated and appropriate. + Performs duties independently with a minimum of supervision but seeks guidance and direction as necessary for performance of duties. + Understands and complies with Title 22 Medicare Conditions of Participation, Medi-Cal, OSHA and other regulations and licensure requirements as they pertain to the position. + Performs other duties as assigned and/or requested. **Qualifications** **Minimum Requirements:** **Experience:** + Minimum of 2 years experience as a PTA in an acute/home care/rehab setting. + Must be a licensed driver with an automobile that is insured in accordance with state and/or agency requirements and is in good working order. **Licensure** : + Current California license as a Physical Therapist Assistant. + Basic Life Support (BLS) + Drivers License + Car Insurance **Overview** Dignity Health Woodland Memorial Hospital is Yolo County's largest health care provider, serving the community since 1905.
