Office Triage LPN
Company: Twin Cities Physicians
Location: Golden Valley, Minnesota
Schedule: Full-Time | Monday – Friday
Compensation: $24 – $27 per hour (based on experience)

About the Role:
Twin Cities Physicians is seeking a skilled and dependable Office Triage LPN to join our Golden Valley team. This role offers the perfect balance between clinical knowledge and administrative coordination—providing phone-based triage and office support that keeps our senior living residents safe, informed, and cared for. As part of a collaborative physician-led team, you’ll help connect residents, providers, and community staff to ensure every care need is met efficiently and compassionately.

Key Responsibilities:

• Triage phone calls from patients, family members, and community care staff to determine next steps
• Communicate and coordinate effectively between clinical and administrative teams
• Process standing orders, review medication lists, and update provider instructions
• Maintain and reconcile patient charts and ensure EMR accuracy
• Respond promptly to phone calls, messages, and faxes in a professional manner
• Support ongoing quality improvement efforts within the office and care teams

Qualifications:

• Current Licensed Practical Nurse (LPN) in the state of Minnesota
• Strong understanding of medical terminology and senior care practices
• Prior experience in a clinic, triage, or geriatric care setting preferred
• Proficiency with PointClickCare (PCC), Eldermark, and Google Workspace (Gmail, Docs, Sheets) a plus
• Excellent organizational skills, attention to detail, and ability to multitask in a fast-paced environment
• Compassionate communicator with a professional and positive attitude
